Adelyn Claudio, RN, Receives DAISY Award

St. Francis Medical Center was proud to name Adelyn Claudio, RN, 6th floor, Telemetry Unit, its most recent DAISY Award winner. The DAISY Award recognizes excellence in nursing. It is a national award and the highest nursing honor bestowed by St. Francis.

Adelyn was recognized for taking the time to know her patients and to ensure that they have the understanding, education, and resources they need to support their healing and recovery. One case serves as a prime example. It involved a gentleman who returned regularly to the hospital for the same health concerns. When he was placed under Adelyn’s care, she was able to learn why. Through patience and discernment, Adelyn discovered the reason why her patient continually failed to comply with his written discharge instructions or to follow through on his prescribed medications, which resulted in his readmissions.

It was when Adelyn was preparing her patient for discharge that she observed his disengaged and dejected manner. She could sense that he was struggling with deeper issues. By taking the time to talk to him, she gained his trust. Before long he revealed to her through tears that he could neither read nor write and that he had no one at home. He couldn’t remember the verbal instructions explained to him in the hospital, and he could not read the written instructions given to him before being released.

As he shared with her further, Adelyn learned about a friend he had in the area. She was able to connect with the friend, as well as with a nearby pharmacy, and she established a support network for her patient. She also carefully explained the important instructions he needed to follow to keep ahead of his illness and out of the hospital. The patient broke down and thanked Adelyn for her help. For the first time he felt different. He felt positive and that he now had a way to break the cycle of readmission.

Nearly a year has passed, and Adelyn’s patient has not been back to the hospital since. As stated by her co-workers in her DAISY Award nomination, “Adelyn’s caring intuition and commitment to taking the time to provide him with what he really needed had a profound impact on this patient and may well have changed his life significantly.”

We thank Adelyn for her exceptional nursing care and the compassion she extends to every patient, every day, and we congratulate her on this well-deserved honor.
